Established in 1999, Case Management Professionals, Inc is a leader in residential care industry   and is one of the pioneer  home and community based case management agency in the State of Hawaii.  .  Case Management Professionals, Inc. is contracted by the Department of Human Services to provide case management services to Medicaid clients who are at nursing home level of care but are cared for in a home or residential facility setting. Case Management Professionals, Inc. also provides consultation and other professional, educational, training and ancillary services to support long-term care needs in the community.  The case management team is comprised of a registered nurse and a licensed social workers.
