Case Management Professionals, Inc. specializes in case management services.  We are a Medicaid contracted provider to provide comprehensive case management for clients who are at nursing home level of care but reside in a home or residential care facility. Case Management Professionals, Inc. also provides consultation, placement, staffing and other professional, education or training and ancillary and contract services to clients and caregivers.

Our staff comprises of highly qualified professionals, with extensive clinical and social services experience and background in meeting each client’s long- term care needs. Each case management team consists of a registered nurse and a licensed social worker.
